This paper examines how temperature variations affect expenditure in South Korea’s Medical Aid program. By merging administrative data at the Si-Gun-Gu level with inter-annual fluctuations in daily maximum temperatures, this paper uncovers that exposure to warmer and colder temperatures is associated with higher expenditure on pharmaceutical and outpatient services, respectively. An additional day with a maximum temperature between 28°C and 33°C leads to a 0.1% increase in pharmaceutical expenditure (approximately 6.6 million KRW), while an additional day with a maximum temperature between 3°C and 8°C leads to a 0.1% increase in outpatient expenditure (approximately 12.2 million KRW). This pattern is driven by increased treatment intensity per patient or case rather than a rise in total visits or patient volumes. In addition, the sensitivity to colder temperatures is positively correlated with heating degree days and medically underserved area status, highlighting regional disparities in climate vulnerability among beneficiaries.
본 논문은 기온 변화가 공공부조 재정에 미치는 영향을 분석한다. 시군구 단위의 연간 의료급여통계와 낮 최고기온 자료를 결합하여 분석한 결과, 고온 및 저온 노출 일수 증가는 각각 약국 및 외래 의료급여 지출의 증가로 이어졌다. 구체적으로 낮 최고기온이 28°C 이상 33°C 미만인 고온 일수가 하루 증가할 때마다 약국 급여 지출은 약 0.1%(약 658만 원) 증가하는 반면, 낮 최고기온이 3°C 이상 8°C 미만인 저온 일수가 하루 증가할 때는 외래 급여 지출이 약 0.1%(약 1,222만 원) 증가하는 것으로 나타났다. 이러한 지출 증가는 진료건수나 진료실인원 등의 양적 증가보다는 개별 진료의 중증도 심화 및 환자별 이용 집약도 증가에 기인하는 것으로 확인되었다. 아울러 난방도일이 높은(평균적으로 추운) 지역이거나 의료취약지역일수록 저온 노출 일수 증가에 따른 지출 증가 폭이 두드러지는 상관관계가 나타나 사회적 취약계층의 기후 취약성에 지역별 격차가 존재할 가능성을 확인하였다.
